Home Health Services: Care That Comes to You

Living in the Weaver Mountains has many advantages.  We have close connections, beautiful surroundings, and a strong sense of neighborly support. However, accessing healthcare can sometimes be a challenge. Long drives, limited transportation, and lengthy wait times can make routine medical appointments stressful or even unattainable for some residents. Unfortunately, when care is delayed, small health concerns can quickly become bigger problems.

This is where home health services can make a meaningful difference. Home health care brings medical services directly to patients in the comfort of their own homes, reducing the need for frequent travel and helping individuals maintain their independence. For many rural residents, this approach to care can improve health outcomes while also reducing stress for patients and their families.

Home health services may include primary care visits, skilled nursing, chronic care management, medication management, routine health monitoring, and coordination of care with other providers. These services are especially beneficial for older adults, individuals managing chronic conditions, and those recovering from illness or injury. By receiving care at home, patients are often better able to stay consistent with treatment plans, recognize changes in their health earlier, and avoid unnecessary emergency room visits or hospitalizations.

Weaver Mountains Health Initiative is pleased to host Alaga Healthcare Group on Wed. February 11th from 1:30–4:30 PM at the Wheelhouse to share more information about home health services and how they can support our community. During this event, Alaga will offer a Medication “Brown Bag” Review with their pharmacist. Community members are encouraged to bring all of their medications or a written list of medications. This service can help identify potential medication interactions, duplications, or concerns, providing a valuable opportunity to understand your prescriptions better.  

In addition, Alaga’s team will be available to provide and discuss a variety of healthcare services, including blood pressure checks, glucose monitoring, blood draws for routine lab testing as ordered by a provider, urine testing and urine cultures, provider visits with a physician or nurse practitioner, nursing assessments, and vaccinations, including pneumonia vaccines and TB screening. These services are designed to help residents better understand their health, address concerns early, and connect with ongoing care when needed.  Sign up for Primary Care at Home, and in-home health services will also be available. Bring your insurance card, Medicare card and your photo i.d.
